Acute Bladder Outlet Obstruction due to Urethral Polyembolokoilomania

Abstract
Background: Polyembolokoilomania involves insertion of foreign objects into body orifices for sexual or psychological reasons. Urethral foreign bodies can cause acute bladder outlet obstruction requiring urgent management. Case Report: A male in his mid-30s presented with acute urinary retention and penile pain after inserting six green peas into his urethra for sexual gratification. Examination showed a distended bladder with palpable foreign bodies along the penile urethra. Attempts at catheterization failed. Emergency cystoscopy revealed impacted peas, which were pushed into the bladder, fragmented using tri-prong forceps, and evacuated. A Foley catheter was placed. Postoperative psychiatric evaluation was normal, and the patient received counselling and psychoeducation to prevent recurrence. Conclusion: Urethral polyembolokoilomania can lead to acute urinary obstruction even without an underlying psychiatric disorder. Prompt endoscopic removal and supportive counselling are essential for optimal management.
