A case report of right diaphragmatic dome agenesis in an adult woman is presented. It is a rare malformation in adulthood because death commonly occurs within the first hours of life. After an unsuccessful open cholecystectomy, the patient underwent an exploratory laparotomy for gallbladder removal, at which point the organ malformation diagnosis was made. Conservative treatment was chosen because the patient was asymptomatic.
