Background: Primary chylous disorder (PCD) of external genitalia is a rare abnormality than can significantly impair patient’s quality of life and psychological well-being. Case Report: We report a 17 year old male with white milky discharge from the external urethral orificium and scrotal skin, noticeable since he was 8 years old. Scrotal exploration revealed multiple enlargement of sub-cutaneous scrotal and peri-urethral lymph vessels. The abnormal structures were excised completely, leaving a bare raw surface of the genital region, which was closed primarily using medial thigh rotational flap. Patient resumed to his normal daily functions within two weeks, and there were no complaints or signs of recurrence reported. Conclusion: Scrotal skin and lymphatics excision is modality to manage PCD involving external genital region.
